Match Officials – Carry Emergency Contact Details.

We advise all Match Officials to carry contact details for an emergency contact who can be reached in the unfortunate event that the Match Official is taken to hospital or requires pitch side medical treatment.

We suggest that all Match Officials insert the following details on to the back page of their Match Record book:

Personal Details

Date of Birth: [INSERT DATE OF BIRTH]

ICE (In Case of Emergency

Contact: [INSERT NAME]

Mobile Number: [INSERT NUMBER]

Landline number: [INSERT NUMBER]

Medical Issues

[INSERT ANY MEDICAL ISSUES THAT THE EMERGENCY SERVICES WOULD NEED TO KNOW]

We further suggest that all Match Official save a contact in their mobile phone as:
